Implement throttling of concurrent processes. While 64 might be a bit conservative in this day and
age, we were exceeding FD_SETSIZE, and maybe we were also lucky. Throttle to 168 for the moment.

Fix handling of prefix length calculation

It was too naive about the mask, endiannes swaps bytes, not bits.
Fix by converting everytihng to big endian, where the mask is
guaranteed to be contiguous set of bits.

Implement no-qbridge mode and workaround for switch mistake

Sometimes switches erroneously use interface index instead of bridge index
in the bridge mib. Workaround by assuming this is probably the case if
none of the macs seem to map to an interface. Note that it's still important
to accept that a bridge port might not have an interface (internal mac address)
so only make the assumption if *none* of the mac addresses mapped to an interface.
